AnonymousInactiveJuly 16, 2012 at 9:26 pmPost count: 14413i have adult add , i am talking methylphidate 20mg SR (sustained release) once in a day.. but it finishes in 8hrs only ..and i have a exam after 3 months which require more than 8 hrs study …pls help me out..what should i do ?? the normal methylphidate’s effect got finish in 3-4 yrs , should i take normal one after SR tablet’s effect finish ? …. please tell me the dose also for normal methylphidate tablet , please help me out guys ..pls..!!

I find that when I’m working flat out, I can sustain it beyond where the medication has stopped working but if you find that doesn’t work for you I know some people supplement with a ritalin. 20mg is quite a low dose (like mine) so I would be tempted to ask for the lowest ritalin dose to try.

They can adjust your dose if needed,according to what works for you. I am not a Dr so I can only offer you support. I wish you the best.
REPORT ABUSEJuly 17, 2012 at 2:11 am #114600I’ve had the same experience on the extended release version. Actually, I just brought this up with my doctor recently. I told her that it kind of peters out very shortly after I come home from work. She felt that that was just fine, seeing as how I “don’t need to concentrate as much at home as at work.”
Actually, I’d much rather be able to concentrate when I’m at home, when I’m not at work, thus have the time to get some writing done.
If only I could motivate and concentrate.
I’ve considered waiting until I get home from work to take my dose. I haven’t tried that yet.
Concentrating at work, I could care less about. I get by fine. It’s a job, not a career for me, so I’m not really invested in being my best self there.
Anyway, just wanted to raise my hand and say that I’ve experienced this too.
As the others have said (and as anyone would hopefully tell you): Always address your concerns to your doctor.

Sadona is correct in requesting you ask your own doctor.. Your doctor has all the necessary history file on you and would be in a better position to assist you..

i am from INDIA, here i couldn’t find any ADD specialist psychiatrist, this ADD problem is very common in general population but its completely undiagnosed and here in india there is very few awareness of ADD. i m doing my medical residency , i finish my medical school last year and i know i have this problem… before posting this topic i already talked with some doctors and told my problems but either they dont have much experience in treating people with ADD or may be something else…basically my doc is chicken , no guts really and the problem is that i cant switch my doc also because he is the only one who prescribe med for me…..
see, the thing is i have a exam after 3 months which require more then 12-13 hrs study …that means i need to concentrate more n more and my methylphidate 20mg SR (sustained release) finish at 7-8 hrs and after that i am not able to concentrate on my studies, so i need some treatment plan which can fulfill my requirements …..
i hope i made myself very clear..
waiting for some early reply….

AnonymousInactiveJuly 17, 2012 at 12:34 pmPost count: 14413I like Concerta 27 mgs. I take it as soon as I wake up and it lasts through supper, so I don’t over eat. Occasionally I take a med vacation (usually unintentionally or when I’ve run out) and that keeps me from building a tolerance. I like the formula too…it doesn’t cause anxiety when it starts to take effect, it works right away (within 10 min) and tapers off nicely. No headaches either. The only downside to these types of meds is that they mess with my dreams, they aren’t as vivid and episodic, like scenes in a movie. Instead, they’ve taken on a gritty quality. My insurance ran out and I’ve had to take generic non-time release tabs for awhile. I can definitely tell the difference. I look forward to getting back on Concerta.

REPORT ABUSEAugust 22, 2012 at 9:34 am #114607Hi deepankar53 – maybe you could try taking a short acting version of methylphenidate when your sustained release is wearing off. Fast release versions last for 3 hours. The two together should see you through your 13 hours.
Alternatively take it later in the day so it lasts later into the evening. Start your day with coffee and then take your meds later? The problem is in my experience, if you are working and studying and sustaining 13 hours day after day, then you’re not ever going to feel refreshed and that will make your symptoms of ADD worse. So together with your wish to work for 13 hours until your exam, you should perhaps think about developing a study programme that gives you breaks and allows you to be more effective because you’re less tired.
On saying that, I know from experience that such as option is not easy. I don’t take more than 60 mg of fast release in a day and I use coffee to top it up if I need to work for an extended period when the effects wear off. I like the fast release though because I can modify the dose to suit what I am doing. I sometimes find it useful to start the day low, double the dose as the day wears on and then lower it again towards evening. Other times, I just take a low dose to tick along, and other times I will take 20 mg three times a day. My psych knows I do this. So I love the flexibility…….but sometimes I forget to take it and crash if the previous dose was a higher one.
However, you do need to be careful that you don’t take too much – I don’t need to tell you that of course if you’re a medic. If you have found someone to give you the medication then hang onto him – such people are not easy to find, even in Europe!
