Hi deepankar53 – maybe you could try taking a short acting version of methylphenidate when your sustained release is wearing off. Fast release versions last for 3 hours. The two together should see you through your 13 hours.

Alternatively take it later in the day so it lasts later into the evening. Start your day with coffee and then take your meds later? The problem is in my experience, if you are working and studying and sustaining 13 hours day after day, then you’re not ever going to feel refreshed and that will make your symptoms of ADD worse. So together with your wish to work for 13 hours until your exam, you should perhaps think about developing a study programme that gives you breaks and allows you to be more effective because you’re less tired.

On saying that, I know from experience that such as option is not easy. I don’t take more than 60 mg of fast release in a day and I use coffee to top it up if I need to work for an extended period when the effects wear off. I like the fast release though because I can modify the dose to suit what I am doing. I sometimes find it useful to start the day low, double the dose as the day wears on and then lower it again towards evening. Other times, I just take a low dose to tick along, and other times I will take 20 mg three times a day. My psych knows I do this. So I love the flexibility…….but sometimes I forget to take it and crash if the previous dose was a higher one.

However, you do need to be careful that you don’t take too much – I don’t need to tell you that of course if you’re a medic. If you have found someone to give you the medication then hang onto him – such people are not easy to find, even in Europe!
